You understand that you should not use Dr. Kiwimed telemedicine visits for medical emergencies or urgent situations. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room.

Who is Dr. Kiwimed?

Dr. Kiwimed is an online medical practice registered in the state of Georgia. Our providers are licensed to practice medicine in several U.S. states including: Georgia, Alabama, Utah, Montana, North Dakota, Delaware, Nebraska, Minnesota, Maine, Texas, Vermont, New Jersey, Illinois, Florida, Arizona, Idaho, Indiana, Nevada, New Hampshire, Oklahoma, Colorado, Iowa, Tennessee, Ohio, Wyoming, Washington and Connecticut. What is a telemedicine visit?

As part of your care, we will conduct a video consultation with a medical provider. Your doctor, nurse practitioner or physician assistant will listen to your concerns and may create a treatment plan if he or she feels it is medically appropriate.

If the treatment involves medication, we may use independent pharmacies to ship your medications, or send the prescription to your local pharmacy. We cannot ship prescriptions to pharmacies outside the United States.

Patient-provider relationship

I understand that the physician or nurse practitioner will decide whether he/she can provide medical care in a responsible manner, based on each patient’s specific condition.

I understand that, in order to be seen, I must create an account with Dr. Kiwimed, complete all pre-visit health questionnaires, have access to video call technology, read and understand this consent, and pay for the visit. I understand that the physician, nurse practitioner or provider has the right to terminate the relationship with the patient if he/she feels the relationship has been compromised.

You understand that you will be treated with respect. You also agree not to be verbally aggressive or disrespectful to the medical team.

Laboratory analysis

  • Laboratory tests may be required in some cases.
  • You understand that the provider may decide whether or not they are required.
  • You understand that lab results are your responsibility to discuss with your primary care physician if they are not related to this consultation.
  • You agree that the provider has the right to make medical decisions and may determine that a treatment is not appropriate, even if you request it.

Important information

  • Requesting a consultation does not imply that a doctor-patient relationship already exists.
  • It may take 72 to 96 hours for a provider to review lab results or messages.
  • The messaging system should not be used for medical emergencies.
  • Nothing on the website, app or social media constitutes medical advice.
  • Completing medical questionnaires does not establish a provider-patient relationship.
  • The provider may decide not to assume your care, even if you mention it on the form.
  • All information on the website or application is for educational and informational purposes only.
  • Use of these services should not take the place of your primary care physician.
  • You are responsible for reading this consent, privacy policies, terms of use and FAQ section before using the services.
  • You should read the medical notes from your visit, medication inserts from the pharmacy and messages sent by Dr. Kiwimed.
  • You have the responsibility to follow up any abnormal results with your primary care physician.
  • You must provide truthful information to the provider and the medical team.
  • If the provider cannot verify your information, it will rely on the information you have provided and assume it to be true.

  • Urinary tract infections
  • Acne care
  • Hair loss/male pattern baldness
  • Sinusitis
  • Acid reflux

  • Cold sores
  • Anxiety
  • Weight loss

These consultations are by telemedicine only, with no face-to-face visits. The provider may recommend that you seek immediate care in an emergency department or consult with your primary care physician.

The telemedicine consultation does not include an in-person physical examination, which means that certain medical conditions may not be detected.

Sending messages or using the service does not imply that you are interacting in real time with a provider. You should check the app or website for new messages. Do not send urgent messages.

Even if the treatment is beneficial, there may be risks of adverse effects or severe allergic reactions.

You authorize Dr. Kiwimed to access your medication history through pharmacy databases. It is also your responsibility to inform the provider of all medications you are taking, including over-the-counter medications.

Treatment risks

You understand that any medication, whether prescription or over-the-counter, can cause serious side effects, severe allergic reactions, permanent disabilities or even death.

You are responsible for making informed decisions about accepting or refusing a treatment plan, considering its risks, benefits and alternatives, including the option of not receiving treatment.
